Public Running List - Completed Community Feedback Forms

This report outlines the status of local PTA units' Community Feedback Forms for continued participation in the 2025-2026 School of Excellence program. These PTAs completed the required form by Dec. 15 and remain in consideration for the 2025-2027 designation. Below, choose "CSV" or "Excel" to download the report.

Create Folder per Application

Use Application Code for Folder Name
Use Application Name for Folder Name (duplicate names will have (1), (2) etc appended)

File Naming

Use Consistent File Naming by Application Name (e.g. ApplicationName-01.pdf, ApplicationName-02.pdf)
Use Consistent File Naming by Application Code (e.g. ApplicationCode-01.pdf, ApplicationCode-02.pdf)
Use Original File Names (duplicate names will have (1), (2) etc appended)
